Housing Consultation is a service designed to assist eligible individuals in developing a person-centered housing plan. Our goal is to help you explore your housing needs, preferences, and goals — and provide guidance on the resources available to make them a reality.
Whether you’re transitioning from a facility, experiencing homelessness, or simply navigating housing barriers, our team will walk alongside you with compassion and expertise.
What We Provide:

✅ Referral Coordination – We’ll refer you to appropriate Housing Transition and Housing Sustaining services if you’re eligible.
Who is Eligible?
You may qualify for Housing Consultation if you:
Are on Medical Assistance (MA)
Are 18 years or older
Have a documented disability, mental health diagnosis, or substance use disorder
Face housing instability or are at risk of homelessness
